What To Bring To Your Appointment
Once you confirm that you are at least 18 years old and have a qualifying medical condition, you can make an appointment with the doctor. You will need to bring the following to your appointment:
- A valid Florida license or state ID
- Proof of your qualifying medical condition
- A copy of your medical records from the most recent doctor visits. These can also be faxed or emailed in advance. If you do not have records of your diagnosis, we can assist you with getting them
- Any imaging studies such as x-rays or MRI’s or the radiology reports of these studies
- Valid form of payment as health insurance programs do not cover this service
- A full-face color photo, passport size, current within 90 days.
How To Use The Medical Marijuana Program In Florida
After confirming that you have one of the qualifying conditions, we assist you in obtaining a Medical Marijuana Identification Card. This card confirms your eligibility to buy medical cannabis within Florida.
The process begins with an in-person appointment with one of our board-certified physicians who is licensed in Florida and certified for compassionate use of medicinal marijuana. At that visit, the physician will review your records as well as complete a focused physical examination. If you are deemed to qualify for
medicinal marijuana under the Florida Statutes, the physician will provide you with a treatment recommendation for the type and kind of medicinal marijuana that will most benefit your condition. This can be any of a combination of tablets, food items, lotions, oils and/or inhalational products.
Our staff will then immediately work with you to apply for your identification card on-site. It is a quick online form that will take you and our staff approximately 10 to 15 minutes to complete. Once the application is submitted electronically, it takes approximately thirty days to receive the identification card. Our staff will contact you when it has arrived at our office and set up a time for pick-up. Once you receive the identification card, you may visit any licensed dispensary in the State of Florida to fulfill your prescription or arrange delivery via a Medical Marijuana Treatment Center.
According to Florida law, you must receive re-certification every 210 days. This re-certification requires an in-person visit with the physician. Any significant change to your qualifying medical condition also requires notification to the physician treating you at Medicinal Cannabis Miami within seven days. The physician can legally certify a cannabis recommendation for a duration of 70 days at a time with refills.
